Senior Living Placement Specialist
Golden Lifestyle Partners provides a personalized service to their clients, family members or legal representatives who are looking for senior living care options. Referrals are made to senior living communities as trusted partners who provide: Independent Living, Assisted Living, Memory Care, and Residential Care/Private Care Homes.
Our personalized placement services include:
In-person Assessment
Consultants gather details on health, care needs, personal preferences, financial situation, and desired location.
Community Recommendations
Senior living communities are identified as best fits based on assessment results.
Tour Coordination
Consultants schedule and, when possible, accompany clients on tours; and act as a liaison between clients and communities
Move Coordination and Support
Coordinate with move managers and provide weekly visits within the first 30 days to support the transition.

Senior Care Consulting
Outside of Senior Living Placement Services, Golden Lifestyle Partners can assist you with additional services including but not limited to the following: nursing home placement needs, transitional care coordination from hospital to home or skilled nursing and rehabilitation care, home care coordination, patient advocacy and ongoing care management. Golden Lifestyle Partners provides senior care consultant services as a private pay service and is not a covered service under Medicare or other insurance plans.
Services may include but are not limited to the following:
Helping facilitate essential conversations between family members
Locating a skilled nursing facility, home care agency or private caregiver selection
Attending care plan meetings
Long distance caregiving
Finding the right resources and learning how to access them
Regular visits to your loved one, providing you with peace of mind when you are unable to visit.
Working with a wide range of experts and local resources, it is OUR GOAL to help seniors receive the care and services they deserve as well as reduce the stress and pressure associated with caregiving.
